Veterinary Vaccine Survey Forms
The Pet Parents’ Action Group is making a public record of veterinary practices with regard to their vaccination policy. You can help by completing a survey about your veterinary practice.
Please print off the forms below and contact your locally veterinary practice. Ask them the questions, complete the form, and either complete the survey form on-line, (or in the UK you can also post the completed questionnaire to us at the address given below). You may complete a questionnaire for more than one veterinary practice (and thank you if you do).
To complete the survey off-line you can download the forms for completion below.
Just remember to tell them to check the vet list page on this site to see if their vet practice has already been included in the survey (we don't want vets to be inundated with duplicate requests for information).
Note for veterinary practices: If details about your practice have been entered incorrectly, please contact us. We will be happy to update your entry. The Pet Parents’ Action Group will also be happy to send a Good Vet Award to practices who have adopted the minimum vaccination protocol.

Tell your friends!
The more people that take part means that we'll be able to get results quicker and be able to follow up quicker. Please tell all your friends, family, associates, club members, groomers etc, in fact anyone who you come into contact with regarding dogs. Email and post details on forums. Tell all your Facebook (or other network) friends - post on Twitter.
The Pet Parents Action Group was launched in the January 2012 CHC Update, which has been made available for free to everyone and has details of the campaign that can be used to pass on to others. For your free copy of the January CHC Update click on the link below (please feel free to pass on as many people as you can):

If you can put up a flyer in your club, in shops, anywhere at all, please click on the file below to print off.

We have also produced a 4 page leaflet that you may find of use to print off to hand out to anyone you think might be interested. Tip: if you have this option when you print, choose the 'booklet printing' option so that it prints as a 4 page A5 leaflet.

Pet Parents' Action Group is looking for volunteers
If you have skills we can use, we want to hear from you.
· Are you familiar with the media, and can you help us get articles in print?
· Do you know about government, and lobbying? Yes? Then you can help the
Pet Parents’ Action Group.
· Are you good at fund raising, or do you know of ethical companies in the pet
products market that might be willing to offer sponsorship? Do you have the
confidence to contact them and ask for their support?
· Do you run dog clubs, or dog shows, or do you hold high office, or have influence,
within a major pet charity or official body? Do you run a training establishment? Are
you willing to get behind this initiative?
· Would you be interested in starting a local Pet Parents’ Action Group branch, and
invite local dog lovers along to share information with them and swell Action Group
numbers?
· Do you have a specialist interest in the dog field? Would you like to form part of a
committee, looking into products, or pet food, or training methods, or
complementary therapies - so that the Pet Parents’ Action Group can make safe
recommendations or issue timely warnings?
· Are you a good researcher?
· Are you a veterinarian, willing to support the Pet Parents’ Action Group?
· Do you run an internet discussion group, or have a great website about canine
health and wellbeing?
Get in touch – the dog world needs to unite and network. We’ll help you, and we’d
welcome your help, too.
